The signer of this agreement has paid a fee for membership at camp designated above. Membership at that camp does not warrant or imply membership at any other camp Licensee.
RENEWAL POLICY : If you renew this membership you will not be requested to pay the enrollment fees over again. If you require changing to another program you need to do so within 15 days of joining. The program can be interchanged only once. You can renew your membership to any program on completion of the earlier one.
NOTICE : Any holder of this consumer credit contract is subject to all claims and defenses which the debtor could assist against the seller of goods or services obtained pursuant hereto or with the proceeds hereof. Recovery hereunder by the debtor shall not exceed amounts paid by the debtor hereunder. If by reason of death or permanent disability, the member is unable to use the services or facilities of the club, he and his estate shall be relieved from the obligation of making payment for services other that that received prior to death or on the onset of disability, and that if her has prepaid any sums for services, so much of such sum as is allocable to service he has not received shall be promptly refunded to him or his representatives. Enrollment fees are non refundable and non-transferable. In case the club is closed or shut down by the management and the services cannot be transferred elsewhere, the management will refund the money on a pro rata basis to the member for the unused period of the contract of membership. You the buyer may cancel this agreement at time prior to midnight on the third business day of the health studio (club) after the date of this agreement, mail or deliver your copy of this agreement and a signed or dated notice or send a telegram which states that you the buyer (member) as canceling this agreement or words of similar effect such as note shall be sent to camp office where you have the membership. By signing below you acknowledge the receipt of the fully completed copy of this membership agreement you and the club and the copy of the rules and regulations. By executing this agreement, you agree to be bound by the terms and conditions contained herein.
WAIVER & RELEASE OF THE LIABILITY : This membership agreement contains a waiver & release of liability and indemnity agreement is section "6" to which you will be bound do not sign this agreement before you read it.
